ABSTRACT

Resumo: Introdução: Este trabalho analisa a qualidade dos prontuários médicos dos pacientes internados no Hospital Universitário Gafrée e Guinle. Métodos: Foram selecionados aleatoriamente 100 prontuários da clínica médica e da clínica cirúrgica de pacientes internados nesse Hospital. A análise dos prontuários consistiu na ausência ou presença, dos seguintes tópicos: ficha de identificação do paciente, laudo médico, boletim de internação e alta hospitalar, resumo de internação, evolução médica, evolução da enfermagem, prescrição médica, boletim operatório, boletim da anestesia, laudo do exame histopatológico. Quando havia estes tópicos, verificou-se ainda se eles estavam adequados ou inadequados. Nos prontuários da clínica médica excluíram-se o boletim operatório e da anestesia, laudo do exame histopatológico e os demais tópicos relacionados à cirurgia. Em cada prontuário foi observado se todos os itens citados estavam presentes, preenchidos corretamente, escritos de forma legível, e se havia abreviaturas, siglas e sinais impróprios. No caso de esses itens estarem presentes nos prontuários avaliou-se a adequação ou inadequação deles. Os resultados das ambas as clínicas foram comparados para a análise de qual serviço apresentou um melhor desempenho. Para avaliar o desempenho do hospital todos os itens foram analisados em relação à presença, ausência, adequação ou inadequação e não aplicabilidade. A análise dos dados foi realizada por meio de tabelas e gráficos e a análise estatística pelo teste não paramétrico Mann-Whitney. Resultados: Durante a pesquisa nem todos os itens foram encontrados e, alguns casos, estavam preenchidos inadequadamente. Além disso, constatou-se a falta de ordenação deles. Do total da amostra analisada, 85,5% estavam legíveis e 32% foram considerados inadequados, sendo 37 prontuários da clínica cirúrgica e 27 da clínica médica. Os resultados dos exames complementares foram encontrados em apenas 55,5% da amostra. Conclusão: A elaboração dos prontuários no Hospital Universitário Gaffrée e Guinle é deficiente com vários itens incompletos e/ou ausentes.


Abstract: Introduction: To analyze the quality of the medical records of patients admitted to the Gafrée e Guinle University Hospital. Methods: A total of 100 medical records were randomly selected from the Medical and Surgical Clinics of hospitalized patients at Gaffrée and Guinle University Hospital. The analysis of medical records consisted in the absence or presence and, if present, whether the following items are adequate or inadequate: patient identification file; medical report; hospitalization and discharge reports; hospital admission summary; medical evolution report; nursing evolution report; medical prescriptions; operative report; anesthesia report; histopathological test report. In the Medical Clinic records the operative and anesthesia reports, histopathological test report and other topics related to surgery were excluded. In each medical report it was verified whether the aforementioned items were present, correctly filled out, using legible handwriting, and whether there were inadequate abbreviations, symbols and signs. The results were compared in both clinics to analyze which service showed the best performance. To evaluate the performance of the hospital, all items were analyzed for their presence or absence, whether were adequate or inadequate and not applicable. Data analysis was performed using tables and graphs, and the statistical analysis was performed by the non-parametric Mann-Whitney Test. Results: Not all items were found nor filled out, in addition to their lack of order. Of the total analyzed sample 85.5% were legible and 32% were considered inadequate, being 37 medical records from the Surgical Clinic and 27 from the Medical Clinic. The results of the additional tests were found in only 55.5% of the sample. Conclusion: The formulation of Gaffrée and Guinle University Hospital medical records is inadequate, showing multiple incomplete and/or absent items.

ABSTRACT

We present a case of primary malignant pylorus neoplasia, emphasizing its rarity, the difficulty of diagnosis and the importance of an adequate prepare before the endoscopic procedure. Literature shows that tumors compromising the first, third and fourth segments of duodenum are rare, especially those of the pyloric area, representing only 0.35% of all malignant tumors of the gastrointestinal tract. Adenocarcinoma is the most common histological type, representing 50% of all malignant neoplasias at this site. The most common symptoms are weight loss, nausea, vomiting, abdominal pain, abdominal mass, sudden changes in intestinal habits and iron deficiency anemia secondary to chronic intestinal hemorrhage. Survival after five years is only 18%. We report a case of a seventy-one year-old male referring early satiety, epigastric pain, retrosternal burning and dyspepsia, with unspecific results in complementary exams, since complete gastric emptying was not achieved. As symptoms worsened, the patient was submitted to laparotomy, with identification of gastric dilation and severe pyloric stenosis, macroscopically suggesting malignancy. The chosen procedure was a subtotal gastrectomy. Adjuvant radiotherapy and chemotherapy were not used. The patient is been followed-up without tumor recurrence so far.

ABSTRACT

Objetivo: Avaliar a ação do ß-1-3 glucan na resposta imunológica de pacientes portadores de cáncer de intestino submetidos à cirurgia, quimioterapia e radioterapia. Método: Estudo retrospectivo de 75 pacientes, analisando as variações dos valores dos leucócitos, linfócitos e subpopulações CD4 e CD8 com o uso de ß-1-3 glucan. Resultados: Houve um aumento no número de todas as linhagens celulares estudadas, principalmente a da subpopulação de linfócitos CD4 (29,70%). A análise estatística pelo teste "t" de Student e pelo de Wilcoxon mostrou que os resultados foram significativos. Conclusão: O uso do ß-1-3 glucan foi capaz de restaurar e manter a resposta imune, reduzir os danos imunossupressores da quimioterapia e radioterapia e o seu uso não demonstrou efeitos colaterais.


Objective: To review the effectiveness of ß-1-3 glucan in the immune response of patients with intestinal cancer submitted to surgery, chemo and radiotherapy. Methods: Retrospective study on 75 patients, analyzing the variations on the leucocytes, lymphocytes and CD4/CD8 lymphocyte subpopulations using ß-1-3 glucan. Results: There was an increase in the general cellular sample, especially on CD4 lymphocyte subpopulations. Statistica analyses via Student's t-test and the Wilcoxon test have showed that the resultshave been significant. Conclusion: The use of ß-1-3 glucan was able to restore and keep the immune response in check, and to reduce the immunosupressor damages from chemo and radiotherapy. There were no side effects.

ABSTRACT

OBJETIVO: Avaliar a ação da timulina em pacientes com neoplasia maligna, submetidos ao tratamento, com e sem quimioterapia e radioterapia complementar (QT/RT). MÉTODO: Estudo retrospectivo realizado em 50 pacientes, analisando as variações das taxas de leucócitos, linfócitos totais e da relação de linfócitos CD4/CD8 após imunoestimulação com timulina. RESULTADOS: No grupo submetido à QT/RT ocorreu um aumento do número de leucócitos após seis meses em 43,33 por cento dos casos, e em 83,33 por cento após 12 meses. Com relação aos linfócitos totais, após seis meses, 63,33 por cento apresentaram níveis maiores, e depois de 12 meses isto ocorreu em casos 90 por cento dos casos. A relação de linfócitos CD4/CD8 mostrou um aumento em 66,66 por cento, e em 90 por cento depois após 6 e 12 meses respectivamente. A análise estatística se mostrou significante com o teste de ANOVA one way. No grupo não submetido à QT/RT a elevação dos níveis de leucócitos ocorreu em 85 por cento dos pacientes aos seis meses e em 90 por cento aos 12 meses. As taxas de linfócitos se elevaram em 60 por cento dos casos em seis meses e em 85 por cento após 12 meses. A relação CD4/CD8 se tornou maior tanto aos seis como aos 12 meses em 65 por cento. A análise estatística mostrou relevância com o teste "t" de Student e o de ANOVA one way. Não houve necessidade de interrupção dos ciclos de QT/RT e nenhum paciente referiu intolerância à timulina. CONCLUSÃO: O uso da timulina foi capaz de restaurar a resposta imune, reduzir os danos imunossupressores e colaterais induzidos pela terapia antineoplásica e não apresentou efeitos colaterais.


BACKGROUND: To evaluate the action of thymus extract (Thymulin) in patients diagnosed with neoplastic disease submitted to surgery treatment, with and without adjuvant chemotherapy and radiotherapy, comparing the results obtained in both groups after 6 and 12 months of treatment. METHOD: Retrospective study on 50 patients, with analyses on the variations of the leukocyte count, total lymphocytes and CD4/CD8 lymphocytes after 6 and 12 months after immunostimulation with Thymulin by subcutaneous via. RESULTS: In the group submitted to QT/RT, there was an increase in the number of total leukocytes in 13 cases (43.33 percent) after 6 months and in 25 cases (83.33 percent) after 12 months. Regarding the number of lymphocytes, after 6 months of thymus extract use, 19 patients (63.33 percent) presented higher levels of lymphocytes, and after 12 months, this occurred in 27 cases (90 percent). The CD4/CD8 lymphocytes ratio demonstrated an increase after 6 months, in 20 patients (66.66 percent) and, after 12 months in 27 cases (90 percent). All these data presented statistical significance with the ANOVA test, being p=0,003 for the number of leukocytes, p=0.0008 for the number of total lymphocytes and p<0.0001 for the CD4/CD8 lymphocytes ratio. Regarding the group that was not submitted to QT/RT, the increase in leukocyte levels occurred after 6 months in 17 patients (85 percent), and after 12 months in 18 (90 percent). In 2 cases (10 percent) the leukocyte levels increased over 100 percent after 12 months. The number of lymphocytes increased in 12 patients (60 percent) after 6 months, and in 17 (85 percent) after 12 months. The CD4/CD8 lymphocytes ratio grew both after 6 and after 12 months in 13 patients (65 percent). The statistical analysis of this sample proved to be important in the evaluation of the leukocytes and total lymphocytes with Student's t-test after 12 months , and it was very significant (p=0.02) concerning the total lymphocytes...
